Application for New Service Connection
Who may apply?
Any interested person, firm, corporation, may avail of the services offered by the Polomolok WD, provided that:
- The service connection shall be installed within the water service area of Polomolok WD.
- The applicant, location, the lot owner / building owner has no outstanding account with PWD.
- The applicant is at least of legal age.
What are the Requirements?
A. Photocopy of proof of lot ownership (any 1 of the ff.)
a.1. Title
a.2. Deed of Sale / Conditional Deed of Sale
a.3. Lot Assessment / Declaration of Real Property or Brgy. Certification (for Gov’t. Lot)
B. Photocopy of any valid ID (any 1 of the ff.)
b.1. Company ID
b.2. Driver’s License
b.3. PRC / SSS / GSIS ID
b.4. Postal ID
b.5. Cedula
C. Cost Estimates
c.1. Attend Orientation (conducted every Wednesday, 9:00 – 11:00 am) at PWD Conference Room
c.2. Fully accomplished Service Application and Form (SACO )
